Rent/Mortgage Financial Assistance

Minimum Documentation Requirements

RENTAL ASSISTANCE:

• Rental / Lease Agreement
  
• 3-Day /30-Day Notice or Eviction Notice - (if applicable).
• 
  
• Rent Payment Receipts (Last 2 months).
  
• Proof of Income/Benefits for all members in the Family Unit.
• 
  
• Photo I.D. and Social Security Card or verification of Social Security number (copy of latest tax return) or other acceptable form of ID.
  
  
• Current bank balances (which can be obtained from bank ATM or on line) & last two months bank statements (Last 3 months up to current date).

MORTGAGE ASSISITANCE:

• Current month's mortgage statement showing Principal/Interest/Escrow Account monies).
• If in arrears, then all documents relating to payments / arrangements, letters / correspondence from lender(s).

• Proof of Income/Benefits for all members in the household.
•  
• Photo I.D. and Social Security Card or verification of Social Security number (copy of latest tax return) or other acceptable form of ID.

• Current bank balances (which can be obtained from bank ATM or on line) & last two months bank statements (Last 3 months up to current date).
•  
• All clients requesting financial or food assistance will be required to complete a Release of Information Statement (form) and a Monthly Income and Budget Form, In addition to completing a complete application packet.
